Bayesian hierarchical nonlinear modelling of intra-abdominal volume during pneumoperitoneum for laparoscopic surgery

Laparoscopy is an operation carried out in the abdomen or pelvis through small incisions with external visual control by a camera. This technique needs the abdomen to be insufflated with carbon dioxide to obtain a working space for surgical instruments' manipulation. Identifying the critical point at which insufflation should be limited is crucial to maximizing surgical working space and minimizing injurious effects. Bayesian nonlinear growth mixed-effects models are applied to data coming from a repeated measures design. This study allows to assess the relationship between the insufflation pressure and the intra--abdominal volume.

Gasless laparoscopy with conventional surgical instruments

The main advantages of gasless laparoscopy arise from the use of conventional laparotomy instruments (clamps, scissors and needle pushers). The operative time, for example, for hysterectomy procedures decreases to about 60% of the time taken hysterectomy for using conventional gas techniques. During cauterization, smoke escapes immediately through the open trocars and may even be suctioned using a conventional suction tube. As far as the nursing team is concerned, the equipment is familiar as they use the same instruments and the same sutures as in laparotomy. Introduction: The Abdominal Compartment Syndrome (ACS) is a “condition in which increased tissue pressure in a confined anatomic space, causes decreased blood flow leading to ischaemia and dysfunction and may lead to permanent impairment of function” Materials and Methods: between june 2007 and june 2008 we treated surgically 23 cases of AAA (14 in election and 9 in emergency), with indirect intra-abdominal pressure (IAP) monitoring (intra-vescical catheter). Mean age was 68 (64-84) years. Mean transverse diameter was 6,2 cm (min 5,5 e max 9,0). Rise in IAP more then 20 mmHg was considered for surgical decompression. Advances in multiparametric brain monitoring have allowed us to deepen our knowledge of the physiopathology of head injury and how it can be treated using the therapies available today.
